COMMUNITY SUPPORT PROGRAM (CSP)

The Community Support Program provides a wide range of services and supports to enable adults with developmental disabilities to live independently in the community. The program assists individuals to develop community-based options tailored to their specific needs and coordinates or provides supports relative to those needs. These flexible supports are coded as 1030 Supported Independent Living and 3000 Community Access and are best suited a person who lives in his or her own home, with no overnight paid support and require between three-five hours of support per week.

Program Goals

  • to maintain Individual accessing services in their community through the provision of practical and concrete supports,
  • to increase Individual’s daily living skills towards greater independence and
  • to increase Individual’s access to formal and informal resources in the community

Outcome Statements

  • Individuals will have access to formal and informal resources in the community.
  • Individuals will have helpful support in a time of crisis.
  • Individuals will have opportunities for social inclusion.
  • Individuals will have life skills development opportunities.

Success Indicators
50% positive change in the Individuals accessing services customized goals between annual intervals. The programs baseline target will be determined in 2012 and used as the outcomes target for change in subsequent years.

Measurement Tools
The Community Support Program will be using a Goal Attainment Scale to measure the successful, partial or unsuccessful attainment of the Individuals accessing services goals.
The Community Support Program will also be participating the Personal Outcomes Index.

Staffing
The Community Support Program has six full-time Community Resource Facilitators that carry an average of 15 Individuals on their caseload. The Community Resource Facilitators are classified as CRW2 position. The Community Support Team is managed by a full-time Senior Program Coordinator.
