SUPPORTED LIVING PROGRAM (SLP)
The program provides individualized supports to adults with developmental disabilities to meet their specific needs and goals. The varied supports include live in supportive roommates, and outreach residential supports and supports towards engaging in individually tailored day alternatives. The program develops proposals, identifying the need and services requested, recruits individual service providers and monitors the support situation. The supports are need specific and reviewed on a regular basis.
All individuals referred to the program must meet Persons with Developmental Disabilities- Calgary Region Community Board’s- Eligibility Requirements.
The program provides individualized support to adults with developmental disabilities to meet their specific needs. Supports and services provided under the program include codes: 1020 – Support Homes; 1030 – Supported Independent Living (including Supportive Neighbor situations) supports; 1040 – In-Home Respite; 1050 – Out of Home Respite; 2010 – Pre-Employment Supports; and 3000 – Community Access.
