The Day Alternatives Program offers comprehensive and cohesive services to meet the needs of adults with developmental disabilities. Individuals accessing services choose activities that are offered mostly in the community and on site generally in a 1:3 ratio. The program operates out of a wheelchair accessible facility in S.E. Calgary. The facility serves as a meeting place for Individuals accessing services and staff to plan activities both long term and daily and as a “safe place” for days that specific Individuals cannot be in the community for various reasons. [due to behavioural concerns, and a center for group activities, courses, etc.]
Services are offered primarily between 9:00 am and 3:00 pm. The primary intent of the service is to assist Individuals to develop skills, knowledge and resources needed to participate in the community as independently and fully as possible.
Supports offered by the program reflect individual circumstances and are tailored to specific areas of need. These supports may be short term (related to a specific task) or long term (to provide ongoing quality of life).
The primary areas of focus include:
- General Education and Interest including some Recreation and Leisure
- Employment preparation and Volunteering
The program will:
- Assist individuals to identify needs
- Develop individualized service plans with goals
- Use “change facilitation strategies” to promote Individuals accessing services success
The Cochrane Community Program (CCP) offers a comprehensive and cohesive day service to meet the needs of adults with developmental disabilities within the Cochrane area. The program operates out of a facility located within the Town of Cochrane. The facility serves as a meeting place for Individuals accessing services and staff to plan activities (long term and daily), a “safe place” for days that specific Individuals cannot be in the community due to behavioural concerns, and a center for group activities, courses, etc.
Services are offered primarily between 8:00 am and 4:00 pm, although some unique activities may occur on weekends and evenings. The primary intent of the service is community focused thus the majority of activities are off site, accessing generic resources and consist of both group and individual activities.
All Individuals referred to the program must meet Persons with Developmental Disabilities- Calgary Region Community Board’s- Eligibility Requirements and can benefit from shared activities within the community.
The program supports the Individuals accessing services to achieve their chosen goals, promotes personal learning, development and independence through their participation in a variety of activities of their choosing. The scheduling emphasis is for these to take place in the community accessing generic resources and activities on-site with the intent of maintenance and or acquisition of a variety of skills and behaviours. Much of the focus is Life Skills and Quality of Life.
Outreach Support Program is a program that helps individuals get involved in the community to do similar activities as the other Day Support Programs (CCP & DAP) The difference from the facility based models is that the support offered in Outreach is one to one (one staff for each individual receiving services). Support hours may vary, according to the needs of the Individual from as little as 10 hours per week, but may be as high as 30 hours per week. The goal of the program is to increase independence and develop social skills, through regular participation in the community.
The Community Support Program provides a wide range of services and supports to enable adults with developmental disabilities to live independently in the community. The program assists individuals to develop community-based options tailored to their specific needs and coordinates or provides supports relative to those needs.
These flexible supports are coded as 1030 Supported Independent Living and 3000 Community Access and are best suited a person who lives in his or her own home, with no overnight paid support and require between three-five hours of support per week.
Program Goals
- to maintain Individual accessing services in their community through the provision of practical and concrete supports,
- to increase Individual’s daily living skills towards greater independence and
- to increase Individual’s access to formal and informal resources in the community
Outcome Statements
- Individuals will have access to formal and informal resources in the community.
- Individuals will have helpful support in a time of crisis.
- Individuals will have opportunities for social inclusion.
- Individuals will have life skills development opportunities.
Success Indicators
50% positive change in the Individuals accessing services customized goals between annual intervals. The programs baseline target will be determined in 2012 and used as the outcomes target for change in subsequent years.
Measurement Tools
The Community Support Program will be using a Goal Attainment Scale to measure the successful, partial or unsuccessful attainment of the Individuals accessing services goals.
The Community Support Program will also be participating the Personal Outcomes Index.
The Agency provides staffed residential supports and day alternatives for adults in group/individualized rural and urban setting(s). The Residential Support Program provides 24 hour care to Individuals with developmental disabilities, including mental health issues, autistic tendencies and/or behavioural concerns. Individuals accessing services present with intense needs and require close supervision to monitor their personal safety and well-being.

Services provided within the program include a home environment. Individuals are engaged in varied on-site and community activities dependent upon their interests, ability and as identified through individual service plans.
These plans are developed for each Individual in a team approach including the Individual, family/guardians, staff, relevant professionals and support networks. Focus of supports is primarily on addressing the Individuals needs for safety and participation in community access at their level of preparedness. As most of the Individuals in need of these services require supports beyond the capacity of traditional day programs, the program strives to provide meaningful day activities tailored to the Individuals capabilities.
COMPLEX NEEDS PROGRAM
The Complex Needs Program is designed to provide long term support to individuals who have a developmental disability and have been identified as having complex needs, posing a significant risk to themselves, others or property. They require intensive services and have, or have had a history of one or more of the following diagnoses or life experiences:
- A mental health disorder
- Difficulty accessing appropriate supports and/or service termination(s)
- Specialized treatment for psychiatric and/or behavioral issues
- Multisystem involvement
- Incarceration(s) or criminal justice involvement
- Chronic substance abuse/dependence problem
Outcome Statements
- Individuals will live in a safe and stable environment
- Individuals will have a reduction in risky behaviors
- Individuals will have valued relationships with others
- Individuals will have access to the appropriate supports and care when needed
Success Indicators
- 80% of positive change in goals related to individuals living in a safe and stable environment
- 80% of positive change in goals related to a reduction in risky behaviors.
- 80% of positive change in goals related to individuals having valued relationships.
- 80% of positive change in goals related to individuals accessing appropriate supports and care when needed.
Measurement Tools
The Complex Needs Program will use a Goal Attainment Scale to measure the successful, partial or not successful attainment in the Individuals personal goals. The Complex Needs Program will administer a yearly Satisfaction Surveys based on the questions in the Personal Outcomes Index currently endorsed by Government of Alberta.
The program provides individualized supports to adults with developmental disabilities to meet their specific needs and goals. The varied supports include live in supportive roommates, and outreach residential supports and supports towards engaging in individually tailored day alternatives. The program develops proposals, identifying the need and services requested, recruits individual service providers and monitors the support situation. The supports are need specific and reviewed on a regular basis.
All individuals referred to the program must meet Persons with Developmental Disabilities- Calgary Region Community Board’s- Eligibility Requirements.
The program provides individualized support to adults with developmental disabilities to meet their specific needs. Supports and services provided under the program include codes: 1020 – Support Homes; 1030 – Supported Independent Living (including Supportive Neighbor situations) supports; 1040 – In-Home Respite; 1050 – Out of Home Respite; 2010 – Pre-Employment Supports; and 3000 – Community Access.
